$82 including postage and packing for USA.

Notes for completing the form: 1. Address, to where the plate will be despatched.
2. Weight, this can be lbs-ounces or kgs, pleae do not use fractions of ounces e.g. 6lb 4½ ozs.
3. Time, please use 24-hour clock to avoid any confusion.
4. Name as it will appear on the plate.
5. We can at £1.00 per word add a brief message on the back of the plate e.g. "From Mum and
Dad".
